Our physicians are participating providers within the TRICARE Program.  If you have a question about your benefits or eligibility, please access Tricare Benefits

Some TRICARE members will require a referral from their Primary Care Manager (PCM) to see a Specialty Care Physician.  Please access https://www.hnfs.net/content/hnfs/home/tn/bene/enroll/pcm/changing_pcm.html for the Specialty Care guidelines for your particular plan.  If a referral is required by your plan, please contact your Primary Care Physician (PCM) immediately to request the referral for your appointment.  All Insurance Referrals should be faxed to (317) 865-8651

** Please note, if you seek specialty care from any provider, network or non-network, without coordinating your care through your PCM or Health Net, you will be utilizing your Point-of-Service (POS) option and will be subject to a deductible and higher out-of-pocket costs.

Before Your Appointment 

Please be sure to bring the following information with you to your first appointment.

  • Primary Care Manager (PCM) Name & Telephone Number
  • Assigned Permanent or Temporary  Duty Station  (Name, City, State and Zip)
  • Assigned Unit - Specify Complete Unit Identification
  • Military Treatment Facility or Clinic Name & Telephone Number
  • Company Orderly  Room Telephone Number


In addition, please fill out our new patient form online and bring a printed copy with you.  This will make the check-in and registration process run more smoothly and quickly.  You may also sign up for an online personal health record and fill out your full medical history so your physician can easily access your records during your appointment.
